- Up to 2 in-person or virtual visits per month
- Direct access to your provider for follow-up questions or concerns
- Same-day or next-day appointments
- Extended appointment times
- Preventive care and wellness planning
- Chronic disease management
- Coordination of specialist care and referrals
- Discounted in-house services
Membership Pricing:
- Individual Membership: $129/month
- Each Additional Family Member: 15% off
- Commitment: 3-month minimum contract
Members also receive 10% off all additional services not included in the DPC membership
